Scoot 'N Zoom (Model #711) Refund Instructions

• Check the bottom of your Scoot 'N Zoom. If you find the model number 711 and it does NOT have a yellow UPC sticker, please proceed.
• Remove all 4 caster wheels from Model #711 (all 4 MUST be returned to process your refund).
• Fill out the below form and submit to receive the recall packet in the mail. This will include instructions for returning the wheels.
  
• Upon receiving your returned wheels along with the completed form, we will issue a check in the amount of $20.00 USD.
• Once you receive your refund, discontinue use of the toy, and discard it in accordance with local recycling standards.
